Click 51-year-old African American woman with spinal gout mimicking paraspinal abscess. MRI of the lumbar spine. [Technique: Sequence: T2, TR: 5664.0 TE: 112.0, Magnet Strength 1.3X, Plane: Sagittal] There is increased T2 weighted signal as well as enhancement in the posterior soft tissues extending from the L1 to the L5 levels. Additionally, there are multiple small fluid collections in the paraspinal muscles at the L3 to L5 levels, greatest on the right. The largest is at the L4 level on the right measuring approximately 11 mm X 7 mm in size, and extending cranio-caudally approximately 2 cm. A smaller collection is seen cranially, measuring approximately 5 X 5 X 16 mm. There is normal alignment of the lumbar spine. The cord signal is normal. The lumbar vertebral heights are preserved. Mild degenerative disc disease is seen at the L4-5 and L5-S1 levels.
